TRANSCRIPT FOR CURRENT STUDENTS and Graduates from the past 5 years

 
To request a transcript, please complete the Transcript Request Form and provide it to DMT in the main office. 
 
Transcript requests can be provided in person or by:
 
1. Mail: Please mail the request, along with a money order to:

Transcript Requests older than 5 years: 
 
Transcripts and employment verifications are done by the Central Records Department for students who graduated more than five years ago.  The phone number for Central Records is: 727-793-2701 x 2500 or visit the website.
